5% Javelin Nova on attack for armor is not present in that list, im not sure if theres something else thats missing

after playing with shrine crafting for some time I realised there are some information (I'm) missing - at first I thought it's as simple as comparing item level to affix's item lvl (from grig's affix table) but then I got high (>=25, even 39) lvl affixes on lvl1 (zomg) gloves and on the contrary I couldn't get lvl >=72 affixes on lvl84 bow.

it seems there is some formula - is the same as in cLOD (see http://wiki.theamazonbasin.com/index.php/Affix_level_chart) so is this formula correct?

on SD forums there's even more complicated formula -they use Ilvl instead of ilvl in Affix level formula:
The "output item level" (Ilvl)

You may have noticed how the Arreat Summit uses "ilvl" for both the magic and crafted item. Although the meaning should be clear to most of you, I cannot stress enough the importance of distinguishing between the two. They are different levels; the ilvl of the magic item to be crafted is used to calculate the Ilvl (capital I) of the crafted item,

Ilvl = int(clvl/2) + int(ilvl/2)

and the affix level formula is:

anybody know what actual highest natural affix % for lightning spell damage is on a belt? Grigs docs suggest up to 30% for ilvl 80, but i've never seen it go nearly that high from regular affixes alone. With crafting that would mean that the highest initial roll you could get for LSD was 45%, is this right? Trying not to waste a bunch of shrines...

grig tools are most likely accurate, and unchanged in ultimative, so yes.
